Druid Grove
Bugbear Assassin
Assassin will kill a Tiefling girl by the telescope.
Encounter is triggered by proximity.
Girl is killed after an in-game timer.
Investigate the Beach
Tiefling child will die if you do not intervene.
Quest is initiated.
Dealing with the Goblin camp. Child will be dead when the tieflings are preparing to leave.
Save Arabella
Kagha will kill Arabella if you do not intervene.
Killed after attacking the Goblin camp.
Save the Goblin Sazza
Sazza will die if you break out Halsin or attack the grove.
Breaking out Halsin or attacking the grove.
Druid Grove Ritual
Ritual will complete, closing the grove off and killing the Tieflings.
Entering the grove.

This sequence begins when you’ve spoken with Minthara.

Blighted Village
The Gnome on the Windmill
The Gnome will disappear.
Proximity.
Trigger is not known, he can disappear after a few long rests.

Find the Missing Shipment
The caravan guards will be killed by Kobolds if left along too long.
The guards will die after a long rest.
Rescue the Trapped Man & Rescue the Grand Duke
Everyone in the burning inn will die if not rescued.

Mountain Pass
Lae’Zel Confronts the Githyanki
Lae’Zel will die if she has left the party to talk to the patrol and you do not join her.
Lae’Zel leaves party.
Triggered if you leave the general area.
Cure the Poisoned Gnome
Thulla will die if not healed or given an antidote.
Triggered by an unknown amount of long rests, or by completing the quest to deal with Nere.
Free True Soul Nere
Nere and a few gnomes will die if you do not clear the rubble in time.
Baldur’s Gate 3: All Illithid Powers
These powers come with huge buffs to critical hits and damage, or grant you unique attacks that do Psychic damage, just to name a couple.
Shadow-Cursed Lands
Don’t Let Rolan Die
Rolan will be found by the southern most bridge in the Shadowfell after leaving the Last Light Inn. He will die if not saved.
The Harpers Ambush Kar’niss
The ambush won’t trigger properly if you delay too long after the harpers leave Last Light Inn. You will not be able to obtain the moonlantern from Kar’niss.
Harpers leave for the ambush.
Long rest or in-game timer.
Lift the Shadow Curse
Playing with Oliver before Halsin rescues Thaniel will avoid this bug. Halsin will die if you leave while he enters the Shadowfell.
Halsin enters the Shadowfell.
Moonrise Towers
Rescue Wulbren & Rescue the Tieflings
You must rescue everyone in the prison by the time you complete the area, otherwise they will die.
Quests are initiated.
Entering the Shadowfell to find the Nightsong.

Your reputation will be damaged if you do not stop the Baldur’s Mouth from publishing.

Cora will be dead at the wine festival if you do not intervene.
Quest is initiated, or entering the Lower City.
An unknown amount of long rests.
Save Duke Ravengard
The Duke can be killed. To rescue him, you must: attend Gortash’s coronation and do not kill Gortash, talk with Mizora and update Wyll’s pact after a long rest, and then go to the Iron Throne prison. Be prepared to rescue the prisoners.
