By naturally progressing the game, citizens will succumb to sickness. Players must craft medicine to cure them. In Whitechapel, Dorothy will sell some medicine. To hand over medicine to an ill citizen press R1 while talking to them.

Upgrade a weapon once then buy a module for it. Modules add special effects and modifiers.

Players can upgrade their weapons in their office in the hospital or at the workbench of safe houses. Upgrade materials are obtained from enemies as random drops, stored in containers in the world, or can be bought from shopkeepers. Once players have entered Chapter 4 and are in West End district, players can buy most upgrade items.

Once players reach chapter 4 they can turn all districts to hostile status. Players should rest until the districts have citizens that have level 3 sickness. Head out and kill a couple of people (4) to drop the district status to under 50%.
